FOTM RULES
If you would like to submit a form to be DJO's Fan Of The Moment, please send an email to webmiss@dymond-jazzel.com. Type FOTM in the subject area. In the email, include the following things:
• Name
• Age
• Location (NOT your address, just state/country)
• Why are you Dymond's fan?
• Message to Dymond (A nice comment to Dymond. No requests for Dymond to call you, come to your party, etc.)
• Picture of yourself (This is optional. If you do send a picture, it will be posted with your FOTM info.)
Note: The site owner reserves the right to choose the FOTM. Any inappropriate applications will be disregarded. If you are chosen as FOTM you will be asked to provide additional information about yourself such as the link to your Myspace page (this is so that Dymond will know who you are).
